Abstract

The number of immediately loaded enosseous dental implants is steadily increasing. In the past, the common loading protocol was considered as prerequisite for successful osseointegration. Today, the success rate of immediately loaded implants has been reported between 88.2 and 100%. Previous studies have led to a precise surgical procedure and have defined prosthetic requirements for a successful immediate loading protocol. In order to reach sufficient primary stability, it is of great importance to use an implant system with adequate primary stability due to the thread design. Implant stability will be successful dependent on the bone quality and the immobilization via primary splinting reducing micromotions and providing stability especially in areas with poor bone quality. The aim of this study was to evaluate the long-term success of 306 immediately loaded implants in different clinical indications including simultaneously bone augmentations with immediate loading. After a mean loading period of 86.04 months, there was no clinical and radiological difference compared to studies from the literature with delayed loaded implants. The clinical parameters demonstrated a positive trend of the immediate loading protocols. The radiological analysis showed that the surrounding bone height of the immediately loaded implants was stable. Interestingly, 31.4% of the implants even had a vertical increase of the bone level. The success rate evaluated in this study was 95.42%. This study provides clinical evidence that immediately loaded implants can be successful in areas with poor bone quality in a long-term. The key to success is, parallel to a standardized loading and recall protocol, the combination with superior patient compliance. Only adequate plaque control particularly around implants can ensure the long-term implant survival and success.
