Abstract
This article describes a six-step model that represents an empirical approach to difficult clinical interactions or impasses in general medicine. The approach is based on an analysis of the problem within the therapeutic system, and therefore includes the physician (observer) as part of the problem. The six steps are illustrated by clinical examples of impasse in patient/physician systems.
